THE LATERAL POTENTIATION OF PSYCHOPHARMACOTHERAPY OF SCHIZOPHRENIA AND OTHER PSYCHOTIC DISORDERS

The relation of psychotic symptoms with pathological activation of structures of right or left hemisphere in schizophrenia and other psychotic disorders, including schizophreniform and schizotypical disorders, are well documented in the literature. Psychopharmacotherapy generally uses neuroleptics and some antidepressants. The action of these drugs is lateralised to some extent, and can be particularity useful. The method of potentiation was used in the course of biological therapy of patients with episodes of paranoid schizophrenia and episodes of schizoaffective disorder. Lateral physiotherapy was used as method of potentiation, including zonal-lateral subsensoral low-frequency electrostimulation by increasing series of negative square-wave pulses of zones in right or left hemisphere. 300 patients were treated by this method with the psychopharmacotherapy. Lateral physiotherapy was used as an independent method of treatment of residual symptoms of psychotic episodes at remission. The therapeutic significant influence of right-hemisphere stimulations was the potentiation of psychopharmacotherapy and reduction psychotic symptoms (p < 0.05). The left-hemisphere stimulations prevented therapeutic action of psychopharmacotherapy, also significantly increasing psychotic symptoms (p < 0.05). Termination of right-hemisphere stimulation reduced psychotic disorders. The speed reduction of productive symptoms was confirmed by a double-blind comparative study, and these effects were accompanied with increased coefficients of variation of latent periods of differentiational visual-motor reactions, suggesting the influence of zonal-lateral electrostimulation on lymbico-reticular structures providing the sensomotor reactions. We also studied the influence of lateral physiotherapy on inter-hemispheric asymmetry of brain electric activity. Overall, stimulation was accompanied by the shift of asymmetry gradient towards activation of the right hemisphere. Monovector character of these changes indicates the effects of zonal-lateral electrostimulation on dyencephalon and lymbico-reticular structures, providing tonic activation of cortex. Our data suggest possible correction of individual profiles of interhemispheral-dyencephalon interactions by lateral physiotherapy, also generally confirming the utility and efficiency of biological therapy of psychoses.
